DVA & Coordinated Veteran's Care Services in Shoalhaven

DVA Coordinated Veterans’ Care (CVC) Program

The Coordinated Veterans’ Care (CVC) Program is a specialised, team-based initiative to provide enhanced support for Gold Card holders with one or more targeted chronic conditions or complex care needs and those at risk of unplanned hospitalisation. This program is designed to improve veterans’ health outcomes and quality of life by facilitating coordinated and personalised care.

Support Services Provided

  • Personalised Care Plan: Your GP, in collaboration with a nurse coordinator and in consultation with you, develops a personalised Care Plan. This plan outlines specific health goals and strategies tailored to your individual needs.
  • Team-Based Approach: Your GP and nurse coordinator oversee and manage your health. They coordinate care across various healthcare providers to ensure comprehensive support.
  • Regular Monitoring and Support: Your nurse coordinator regularly contacts you to monitor your health status and progress towards your health goals. They provide ongoing support, answer questions, and address any concerns you may have.
  • Feedback and Appointment Assistance: The nurse coordinator provides feedback to your GP regarding your health progress. They also assist in scheduling appointments with other healthcare professionals, ensuring timely access to necessary services.
